Fall 2013 Registration Info
Players must be 4 years old prior to August 1st 2013 to be eligible to play Fall 2013.
AYSO 574 has gone Paperless!
To complete the application process, all parents are asked to use the following process:
· Use the instructions “eayso registration instructions.pdf” to enter your player information at www.eayso.org.
· Please note that AYSO now is offering “E-Signature” to help reduce paperwork.
· Entering your player(s) information in eAYSO only completes the application portion of the process.
· To complete the registration process it is necessary to mail the following information to the address below:
o If you used eayso “E-Signature” bring (1) of the registration form
o If you were unable to use eayso “E-Signature” please bring (2) Signed copies of the registration form.
o Payment of $55.00 per player with check payable to: AYSO 574

All Registrations postmarked by May 31, will be eligible for the $55.00 "Early Registration" fee. After “Early Registration” ends fees will increase to $80.00 per player.
